This requirement is for the department of Health Canada.

Title: The Development of a Health Canada Learning Program on Chemicals and Human Health

Introduction:     

The Contractor will be expected to design and develop a Health Canada Learning Program on Chemicals and Human Health, consisting of several distinct modules and an accompanying instructor’s guide. The learning modules are to be based on existing Health Canada publications, materials, and other relevant information related to the Government of Canada’s Chemicals Management Plan (CMP). The draft product will be piloted by Health Canada and the pilot test results will be used by the Contractor to develop the final materials. This Health Canada Learning Program on Chemicals and Human Health will be used by Health Canada staff or their designates to raise awareness of chemical substances among Canadians and to encourage them to make informed decisions to better protect their health.

Estimated Value:


The total value of any contract resulting from this RFP shall not exceed $60,000 (this amount includes $55,000.00 for professional services and $5,000.00 for travel) including travel and living expenses (if applicable), other expenses and all applicable taxes.

Mandatory Requirements:

M1 Bidder Profile and History

1. The Bidder MUST provide a corporate profile demonstrating:

a) The full legal name of the entity submitting the Proposal and identification of all proposed resources within the Proposal, including, as applicable, all joint venture or consortia members, partners or subcontractors;

b) An organizational chart and brief description of the Bidder’s management structure as it relates to this requirement, including decision-making processes, accountabilities and reporting relationships between the Bidder’s operating divisions;

2. The Bidder MUST demonstrate their capabilities in conducting lesson plan, curriculum development, and material design as described in the statement of work by providing three (3) written project summaries, describing in detail of the Bidder's current/previous experience in successfully providing services on client projects of a similar nature (i.e. environmental health) to HC’s current requirements as defined in the Statement of Work (section 2.0) of this RFP during the past five (5) years from date of this RFP.
            
3. Within each project summary provided, Bidders MUST include the following information:

a) The name of the client organization to whom services were provided;

b) a brief description of the project, including the type and scope of services provided (e.g., target audience, learning objectives, approach for the design and delivery of the project);

c) Project duration, including start and finish dates. (dates should be identified by month, day and year – for example March 15th 2004 – February 15th 2007);

d) The dollar-value of the project (to the Bidder);

e) The extent to which these services were provided on-time, on-budget and were effective at producing desired learning outcomes; and

f) The name, title, and valid contact information including: telephone number, fax number, or e-mail address of a Client Reference within the client organization who possessed oversight or approval authority over the Bidder’s work for the cited project.

i) The named individual identified as a project Client Reference MUST be a resource affiliated with the identified client organization to which the work was delivered, and not a member or affiliate of the Bidder’s organization or Joint Venture Partner organization, or of a sub-Contractor of the Bidder.

ii) The named individual identified as a project Client Reference MUST have held a position of authority within the client organization for the work undertaken by the Bidder in relation to the referenced project.

iii) Health Canada reserves the right to contact the identified project Client Reference to verify the accuracy and veracity of the information provided within the Bidder's Proposal with respect to the client project. In the event Health Canada chooses to contact the named client reference and the client reference provides a negative response with respect to the accuracy or veracity of the information provided within the Bidder's Proposal, the Bidder's Proposal will be deemed non-compliant with no further consideration given thereto.

iv) Health Canada also reserves the right to reject a bidder should one or more of the references provided not be available within one week from completion of bid evaluation

That the Bidder has conducted lesson planning design and curriculum material development activities in Canada similar to those described in the Statement of Work within the past five (5) years from date of this RFP
