Description

The purpose of this Request for Proposal (RFP) is for the procurement of the pooled benefits plans for the Community Health Ontario Group Insurance Plan (CHOGIP), the umbrella organization for the merged group insurance plan for the Association of Ontario Health Centres (AOHC), the Addictions and Mental Health Ontario (AMHO) and the Association of Family Health Teams of Ontario (AFHTO).

***** IMPORTANT PRE-QUALIFICATION INSTRUCTIONS*****

In order to obtain the RFP package, the proponent’s authorized representative will be required to sign a Confidentiality Agreement and a Declaration confirming your organization is a FSCO licensed Life Insurance company which can underwrite Life, Accident and Sickness insurance in the Province of Ontario, at the time of release of the RFP documents.

The Confidentiality Agreement and Declaration Form will be made available on MERX and the completed document must be submitted by e-mail to the Client Representative as identified on the form. Upon receiving the successfully completed form, The Client Representative will e-mail the RFP package to the authorized representative.

The proponent must meet the pre-qualification requirements in order to participate in the RFP process.
