Submission Requirements:

Submission Form (Appendix E)
Response content for rated criteria (Appendix C)
Provide proof of current professional liability (errors and omissions) coverage as referenced in Appendix A, Schedule 2-B.
Provide proof of current commercial general liability coverage as referenced in Appendix A, Schedule 2, 8.02.
Submit a valid clearance letter from the Workers' Compensation Board (WCB - Alberta) stating that the Respondent has an account in good standing with the Board.
Contractor to provide a completed Proof of Vaccination Declaration Form included as Attachment 3.
Provide at least one of the following:
1. Verification of a valid Certificate of Recognition (COR) or Small Employer Certificate of Recognition (SECOR) with submittal OR
2. One (1) copy of current Health and Safety Manual as acceptable by CHC and in compliance with Alberta Occupational Health and Safety Act
