This RFP is being issued by Bellwoods Centres for Community Living to obtain Proposals for the provision of Personal Support Workers Agency Services from Proponent(s) in the Greater Toronto Area (GTA).
**Personal Support Worker (PSW) services refer to the provision of basic care services to persons with physical disabilities who require assistance with daily living activities.
The Proponents must have the ability to cover the total estimated hours required by Bellwoods as listed in Section 1.6 – Current State and must have the ability to provide the full scope of services as indicated in Schedule D- Description of Services.
The objective of this RFP is to select and enter into Agreements with up to three (3) Service Providers who can meet or exceed Bellwoods Personal Support Worker requirements on an as needed basis, through the provision of the services as described within this RFP document.
All personnel supplied by the Agencies shall provide an equivalent level of care and treatment to clients that Bellwoods would expect from their own employees. Agency supplied personnel is expected to be compliant with Bellwoods internal policies, procedures and systems as well as in compliance with all legislated and professional regulations.
The proposed term of the Agreement will be three (3) years plus a two (2) year renewal option, which may be exercised one (1) time.
Due to the nature of the bid solicitation, bids transmitted by facsimile to Bellwoods will not be accepted.

The Mandatory criteria are:
(i) Demonstrate Accreditation with an Accredited body.
(ii) Certificate of good standing (clearance certificate) by the Workplace and Safety Insurance Board issued within six (6) months previous to the closing date of this RFP.
(iii) Information about the Service Provider’s current fiscal viability and performance such as the most recent audited financial statements.
(iv)Provide and maintain a web‑based staffing and scheduling software to coordinate requests for Support Services from Bellwoods, to assign Service Provider Personnel to fulfill such requests, to track the status of Support Service requests; to generate invoices and reports, to geo‑track Service Provider Personnel for the purpose of confirming attendance at Scheduled Shifts.
