Additional Bidding Instructions

will consist of one (1) original hard copy of the Technical Submission
that includes the following Section 1.0 Mandatory Submission Requirements - 1.2 Bid Bond (Section
00 43 13), 1.3 Agreement to Bond (Section 00 43 14) and 1.4 List of Similar Contracts Completed
(Within Last 5 Years) (Section 00 45 13) appended as ‘Section 1.0 Mandatory Submission
Requirements’ immediately following the Technical Submission cover page.

The Bidder’s response to all items in 4.2.3 Rated Submission Requirements shall be appended as
‘Section 2.0 Evaluated and Scored Submission Requirements’ of the technical submission.

The Technical Submission is to be submitted in a binder with the Bidder’s company name, the RFB
number and “Part 1 - Technical Submission” clearly indicated on the binder cover.

As outlined in the contract documents.

Envelope 2 - Pricing Form

Additional Bidding Instructions

must consist of one (1) fully completed original hard copy of Mandatory
Form 1.1 Bid Form (Section 00 41 00), signed (where applicable) by a representative of the Bidder
who is legally authorized to bind the Bidder’s firm. An incomplete form and/or forms which have
had alterations made to the original form format or content may be subject to disqualification.
