Calgary West Central Primary Care Network is currently accepting proposals to provide a Curriculum Development and Design Consultant for members of the organization’s Patient Medical Home (PMH) team (e.g. allied health professionals, measurement and improvement professionals). Calgary West Central Primary Care Network is a non-profit, health care organization, with a network of medical professionals. With 41 functioning Primary Care Networks in Alberta, Calgary West Central represents one of such networks.
The objective of the new curriculum is to provide robust, consistent and effective training to ensure that the PMH team delivers collaborative, patient-centered care and uses evidence-based care plans that incorporate insights from data and professional expertise.
Accompanying this letter is the Request for Proposal for Curriculum Development and Design package which includes a full introduction of Calgary West Central Primary Care Network, our business as well as the requirements for proposal submission in electronic format. A notice of intent to bid is due back to the Calgary West Central via e-mail no later than Friday, December 14, 2018. Proposal submissions are due via email or mail, no later than January 4, 2019.
