A completed copy of Price Schedules “A” and/or “B” (pages 5 and 6); □
A completed copy of the electronic Vehicle Identification Form (separate document, available through MERX);□
A scanned / photographed copy of the complete original ownership(s) papers for all vehicles offered;□
A valid copy of Commercial Vehicle Operator’s Registration (CVOR) Certificate (Or Equivalent) that is active and valid as of the Closing Date of this RFSO (see Section 45); and□
A copy of City of Ottawa Truck Box Measurement For Snow Removal Ticket(s) issued by Fleet Services (see Section 38 Measurement Confirmation).
