CC-LB-0001-MB - RFP for Glucose Meters – Hospital Grade Units, Handheld Units and Consumer Grade

This Request for Proposals (the “RFP”) is an invitation by the Coordinating Entity to prospective proponents to submit proposals for the Provision of Glucose Meters – Hospital Grade Glucose Meters, Handheld Grade Glucose Meters and Consumer Grade Glucose Meters for the Province of Manitoba, as further described in Section A of the RFP Particulars (Appendix D) (the “Deliverables”).

The Coordinating Entity, as identified on the cover page, is issuing this RFP and administering the RFP process on behalf of the Purchaser(s) identified in Section B – Material Disclosures of the RFP Particulars (Appendix D).
This RFP is broken down into three (3) separate categories for award: Category 1, Hospital Grade Glucose Meters, Category 2, Handheld Glucose Meters and Category 3, Consumer Grade Glucose Meters. Shared Health reserves the right to award these categories separately to multiple proponents or one successful proponent for all. 

For Categories 1 and 2, the Request for Proposal will be searching for a Company to provide a “Lease to Own” model where the equipment is purchased through consumable milestones.
For Category 3 the Request for Proposal will be searching for a Company to provide product to be purchased.
