Attachment # 1 – Mandatory Requirements
As per Article 2.2.2 of this RFSQ, the following mandatory forms, information and attachments must be completed and submitted in Attachment #1 – Mandatory Requirements:
• M1 - Appendix A – Prequalification Submission Cover
• M2 - Appendix B – Prequalification Submission Declaration Form
• M3 - Appendix C – No Conviction of Fraud Declaration Form
• M4 - Appendix D – Conflict of Interest, Ethical Conduct and Disclosure of Information Declaration Form
• M5 – Appendix E - Evidence of Insurability or valid insurance certificate
• M6 - Copy of Workplace Safety and Insurance Board coverage (WSIB)
• M7 – Consent of Surety

Attachment #2 – Technical Rated Criteria
As per Article 2.2.3 of this RFSQ, the following forms, information, and attachment are to be included in attachment #2 - Technical Rated Criteria:
• 1 – Annex I – Company Profile and Organization Chart
• 2 – Proposed team for the project (narrative and resumes for proposed Project Manager and Superintendent)
• 3 - Annex II – Company Relevant Experience and References
• 4 – Annex III – Approach, Methodology and Quality Control
• 5 – Annex IV – Health and Safety
