Description

This Request for Proposal (RFP) is being issued by Fraser Health Authority, (FHA) to seek responses from qualified service providers with a proven track record to provide services on behalf of Fraser Health Authority, for Health Contact Services (HCS). This can include but is not limited to a fixed site location health contact centre model within the City of Abbotsford. It could also include other program model types such as mobile, virtual or nomadic. Any of the proposed services, regardless of model type, must include witnessed consumption components (witnessed injection and inhalation where possible) for the City of Abbotsford as defined.
