COL-23-61 - Senior Support Navigator

The Seniors Community Services Partnership (SCSP) network was formed several years ago and created partnerships between six organizations committed to providing services for seniors. Services include Senior Systems Navigation, In-Home Support, Volunteer recruitment, and counseling services. The organizations involved are Lethbridge Senior Citizens Organization, Canadian Mental Health Association, Lethbridge Housing Authority, Nord-Bridge Seniors Association, Lethbridge Family Services, and Volunteer Lethbridge.

The City intends to contract a service provider to employ a Senior Support Navigator (SSN), who will provide seniors with connections to the continuum of care, referrals, and follow-up support to aid in individual wellbeing. The SSN will work in collaboration with the SCSP, and all other Community Social Development (CSD) funded service providers as a population specialist for senior citizens. The SSN is responsible for working within the Integrated Coordinated Access system (ICA), and other key social service agencies to review and support services for senior-specific system navigation and referrals. The SSN will work with ICA staff to be trained as a Tier 3 level navigator and support the ICA system as such.
