Description

Ontario’s Primary Care Action Team (PCAT) will implement a Primary Care Action Plan supported by the government’s investment to connect two million more people in Ontario to a family doctor or primary care team within four years.

The Primary Care Action Plan will help implement a broad series of initiatives in collaboration with primary care leaders and health system partners across three pillars:

i. Connecting You to a Primary Care Team
ii. Making Primary Care More Connected and Convenient
iii. Supporting Primary Care Providers

Ontario Health is seeking services of a vendor with a team of qualified experts with clinical expertise to provide strategic advice and recommendations to support achieving the above mentioned pillars of the Primary Care Action Plan, and to help build a comprehensive, convenient, and connected primary care system.
