AB-2025-04779 - Request for Proposals - Services to Alternate Level of Care PDD Eligible Complex Individuals

Assisted Living and Social Services (ALSS) provides support for seniors, families, and communities, and helps Albertans access disability services, financial supports, services for the unhoused, and other social-based programs. The Province supports Albertans through a person-centered, integrated service delivery model that recognizes the unique circumstances, experiences and strengths of individuals and families. As part of the Disability Services (DS) division within ALSS, Disability Services, Persons with Developmental Disabilities (PDD) program funds services to help adult Albertans with developmental disabilities to be a part of their communities and exercise self-determination to live as independently as they can. These services supplement the support of family, friends, community members and available community resources or government programs Services are provided to support individuals in their home and social environments based on the individual’s assessed support needs. The division is seeking interested proponents to support highly complex PDD individuals that are considered Alternate Level of Care (ALC). ALC refers to a situation where an individual in a hospital setting no longer requires the intensity of care provided and is awaiting transfer to a more appropriate setting. This situation arises due to complexity of service needs. In addition to requiring staffing supports and services, these individuals also require housing solutions which often includes modified living arrangements due to complexity of behaviors. The division is seeking to fill service provider capacity across the province and anticipates that between 40 - 50 individuals will be in a position to transition out of hospital or hospital like settings by September 2025.
