Project Background Details:
The C-Arm will be used in the course of research involving large animal subjects.
Scope/Project Details:
This RFP is being issued by Research Facilities to obtain Proposals for the Hospital for the supply and installation of a new C-Arm. Bidders should also provide warranty, the equipment’s standard manufacturer’s warranty PLUS extended warranty that would total at least (5) years for parts and labour of all products offered. The first 2 years should be included in pricing, with the remaining 3 years being priced on an annual basis.
The pricing for the extended warranty, including all identified requirements, should be listed separately for each of the years. Pricing should be submitted as separate price line items for each year of service as year one (included), year two (included), year three (price), year four (price) and year five (price).

Bidders are permitted to provide in their response(s) options for equipment enhancements or service however those options will not be evaluated. Those options may be considered for future requirements.
Scope of Work – Details:
Scope of Work - Supply of Equipment:
a) Provide a detailed outline of your plan and schedule for delivering the Equipment including tasks, milestones and timeframes. Also include estimated lead time during the agreement and variations in the lead time, if any.
b) Describe how the Proponent will be able to maintain an adequate supply of consumables, parts, and services for appropriate maintenance of the proposed Equipment for the duration of the Agreement.
c) The Proponent should provide details regarding any cost reduction initiative the Proponent considers could be implemented immediately upon execution of the Agreement or during its first year, explaining the recommended processes to be implemented, including the benefits.
d) Proponent should provide details on available resources for moving, lifting and setting up of the equipment as specified in this RFP.
e) Proponent should provide installation and operation manuals upon delivery of the equipment.
f) Proponent should confirm compliance to Acceptance testing period of 15 days/weeks as indicated in the RFP.
g) Proponent should conduct a site –visit to verify site condition, if requested by UHN, prior to delivery and set-up of the equipment.
h) Proponent and Proponent’s sub-contractors should be prepared to coordinate project timelines and work with UHN Research Facilities Planning Project Manager by providing detailed schedules covering delivery, installation, validation, training and acceptance testing.
Scope of Work – Disaster Recovery and Business Continuity Plan:
a) Describe the guaranteed response time to assess recalled Equipment that has been identified by the Purchasers.
b) Describe the resources available to assess all recalled equipment to ensure the Equipment meets current safety standards and regulations.
c) Disclose and describe discussions that the Proponent has had, is presently in or will be in process with any regulatory agencies related to the Equipment which may impact on the future availability of the Equipment, accessories, or service support of the Equipment.
d) How does the Proponent ensure that the operations of the Proponent and its clients continue when disrupted by natural events, labour unrest and facility emergencies?
e) How does the Proponent ensure the supply of Equipment in the event of natural events, labour unrest and facility emergencies at manufacturer plant or site of operations?
f) How does the Proponent ensure that adequate efforts will be made by the Proponent as well as the manufacturer to reach the Purchaser premises in case of natural events, labour unrest and facility emergencies at Purchaser’s location?

Scope of Work – Site Planning Schedule:
a) The Proponent should provide detailed information on all utility information and requirements, including all power requirements for the system (regular or emergency) and plans to utilize existing UPS, or identify any need for additional UPS unit(s).
b) The proponent should specify any computer data and networking requirements (data ports, bandwidth etc.).
c) The Proponent should identify and state the effect of power outages or transitional interruptions experienced upon transfer to emergency power generators, together with the significance of interruption of any other supplies.
d) The Proponent should provide detailed information on all air conditioning, venting requirements and recommendations on the type of cooling required for the equipment, scan room, the control room and any other areas affected by equipment heat discharge.
e) The Proponent should indicate the total heat generated from the equipment, expressed in BTU’s per hour for each section or portion of the system(s) housed in separate rooms.
f) The Proponent should provide detailed information on all plumbing, water treatment and water flow requirements.
g) The Proponent should outline their requirements for an Electrical Power line conditioner and/or specify their equipment tolerances with respect to line fluctuations.
h) The Proponent should identify arrangements for planned interruption of services for connection of equipment.
i) If domestic water is used for cooling purposes, the Proponent should provide a design plan for reduction of waste. Note: This should be used as a last resort for cooling and part of the installation should include a measuring device(s) for water consumption specific to the project that may be connected to a Building Automation Control System.
j) The Proponent should identify all other Site preparation requirements that are out of the Scope of this RFP, however, are required prior to Equipment’s Installation.
k) The Proponent should provide any requirements for lead or magnetic shielding for the space.
l) The Proponent should provide the weight and dimensional size (shop drawings) of the proposed equipment.
m) The Proponent should confirm that all Site preparation requirement(s) being performed will be in accordance with CSA Z317.13-07 Infection Control for Construction and Renovation.
n) Proponent should include proof of insurance, WSIB documentation, company’s health and safety policy and be prepared to attend a minimum of one (1) hour safety orientation at UHN’s request.
o) Electrical: The Proponent should confirm that electrical components will meet the requirements of CAN/CSA-Z32-04.
p) The Proponent should identify the number of emergency power outlets that will be required, if necessary.
q) The Proponent should identify ideal operating condition and environment for the proposed Equipment.
r) Medical Gas: The Proponent should provide a list of medical gases required for the successful operation of the proposed Equipment, if required.
s) The Proponent should indicate the maximum number and type of outlets and any other auxiliary connectivity requirements to be provided for the medical gases, if required.
t) Testing and Inspection: The proponent should provide information on the Equipment’s inspections and certifications.
u) The Proponent should indicate if the proposed Equipment is sensitive to vibration, electromagnetic field, radio frequency, electrostatic or any other sensitivity.
v) The proponent should confirm that proposed equipment meets Good Laboratory Practices. ( http://www.hc-sc.gc.ca/cps-spc/pubs/pest/_pol-guide/dir98-01/index-eng.php).
w) The proponent should provide documentation or certification that the proposed Equipment meets all industry-required safety specifications.

Scope of Work – Training and Education:
a) The Proponent should provide a list of all proposed training schedules including the description, length and scope. Applicable l training should include all aspects of the Equipment including, but not limited to equipment operation, routine maintenance, preventative maintenance, cleaning, disinfecting and trouble-shooting of all components.
b) The Proponent should provide all training manuals, materials, documentation, installation guides.
c) Ongoing: The Proponent should list and describe on-going training opportunities (e.g. refresher courses, seminars, multi-media materials, in-service sessions, etc.) provided by the Proponent.
Scope of Work – Services and Support Exhibit:
a) Warranty – The Proponent should provide a minimum of two years standard manufacturer’s warranty, the coverage should include; onsite warranty, spare parts, repair parts, labour, travel expenses, unlimited service calls, and annual or bi annual maintenance.
b) Post Warranty Service – The Proponent should provide post warranty terms identical to the standard manufacturer’s warranty upon its expiration. The Proponent should disclose expected end of life support duration.
c) Preventative Maintenance: Hours of Service - The Proponent should indicate the recommended frequency, type and length of procedures for appropriate preventative maintenance of all proposed Equipment.
d) Documentation - The Proponent should provide written documentation for the recommended preventative maintenance on the Equipment including but not limited to cleaning, calibrating, periodic replacement of parts, and performance verification.
e) Response Time - The Proponent should indicate the guaranteed on-site service response time of a qualified repair technician after a request has been made. This information should be provided for: regular weekdays; weekends; public (statutory) holidays.
f) Technicians - The Proponent should indicate the number of trained service technicians at each location and identify locations in Ontario.
g) Parts - The Proponent should indicate the Parts depots located in Ontario and indicate: a list of parts to be kept on site (consignment); a list of tools on site; and normal lead time for parts.
h) Telephone Support: Availability and Services - The Proponent should describe in detail the telephone support available during and after warranty and indicate availability including but not limited to: 24 hours per day, 7 days per week, 365 days per year; operational/application inquiries; remote diagnostic troubleshooting and user networking; and a toll free line.
i) Length of Support - The Proponent should indicate the length of time that technical telephone support will be provided to the Purchasers (i.e. as long as the Equipment is in service at the Purchasers) and identify any associated costs.
j) Remote Connectivity - The Proponent should provide details of Purchaser obligations regarding remote connectivity if applicable.
k) Loaner Equipment: Availability - The Proponent should indicate if loaner equipment is available if required, for use when existing Equipment requires repair, maintenance, recalibration, etc.
l) Turnaround Time - The Proponent should indicate the guaranteed turnaround time to receive such loaner equipment from the time the initial request is made.
m) Guaranteed Uptime - The Proponent should provide the guaranteed uptime for the equipment. The minimum acceptable guarantee is 98%. In the event the Proponent is unable to maintain the guaranteed uptime, the Proponent should include how the warranty or service contract would be extended at no charge.

Scope of Work – Environment Considerations:
a) Proponents should submit energy and water consumption information, including any power saving options, as part of the equipment specifications. Information should be in energy units (i.e. not $).Proponent with the most energy saving equipment will obtain the maximum points). If already included in specification sheets to be submitted, the Proponent should highlight the relevant sections with energy info.
b) Proponents should state whether this Equipment or its component contains any mercury (in switches or thermostats etc). If yes, state estimated quantity. Proponent with the least or no mercury content will obtain the maximum points.
c) Proponents should describe whether their company has a trade in discount for old equipment (end of life/ take back/ recycling program). Proponent with the best trade in discount in monetary value will obtain the maximum points.
d) Proponents should state if their companies are involved in sustainability auditing: ISO 14000, ISO 14031, Natural Step, Triple Bottom Line Accounting (Maximum Points will be awarded to Proponent participating in two or more of the above stated initiatives).
e) Is the proposed equipment energy efficient? Explain.
f) Does this proposal include large piece(s) of equipment (e.g diagnostic imaging equipment)? If yes, specify equipment’s footprint.
g) The Proponent should provide the number of Equipment to be installed and indicate Energy Star rating(s) of all proposed Equipment.

Acceptance Testing – Details:
Acceptance testing will be conducted over a period of 30 days.

Certifications If Applicable:
Proponents must provide Installation Qualification/Operational Qualification (IQ/OQ) certificate(s) for attestation at the installation site.
All equipment must be compliant with Canadian Standards Association (CSA) regulations.

Tiebreak Criteria in case of a Tie Score:
In the case of a tie score the Bidder with the highest score for the scientific specifications will be awarded.

Specifications:
For the purposes of the requirements stated in this RFP
A. “must” and “shall” indicate that the requirement is mandatory, subject to provisions of this RFP; and
B. “should”, “could” and “may,” indicate that the requirement is discretionary.
The specifications must address both mandatory requirements and general requirements as applicable in accordance with the scientific requirements that the researcher needs to perform the scientific research. Note: Optional Requirements – Cannot Be Evaluated.
Mandatory Requirements:
The C-Arm must have the following:

a foot pedal/switch for controlling fluoroscopy;
a switch/control button on the control console for controlling fluoroscopy;
a dose area product meter for purposes of dose monitoring;
user controlled anatomical program settings.

capability to feed a video signal to an external monitor;
capability of capturing a single exposure to archive
image receptor is to cover, or be configured to give fields of view of approximately twelve inches in diameter and nine inches in diameter;
collimator and shutters to limit the imaging field;
ability to add markers and/or comments to the image;
ability to work in conjunction with the Fluoro Nav Surgical Navigation System,

Medtronic "Fluoro Nav" surgical navigation system, Brainlab, Stryker remote control functionality;

minimum internal hard disk storage capacity for 1000 static or 15,000 dynamic images;
capability and flexibility to reduce subject radiation exposure (i.e filtration functionality
capability of supporting DICOM modality work list management, including, but not limited to:
DICOM Storage Service Class (SCU)
DICOM storage commitment push and pull
DICOM Modality Performed Procedure Step (MPPS).
all network connections are to support a minimum transfer rate of not less than 100Mb/s (i.e. Fast Ethernet).
capability of transferring images to USB drive.
The Supplier will be required to assist the Hospital with interfacing the Equipment to a Radiology Information System (RIS) and Hospital Information System (HIS) as selected by the Hospital.
For complete clinical acceptance the DICOM interconnecting of the system with existing system as described must be demonstrated to be fully operable. The Supplier must provide qualified and knowledgeable personnel to assist with this task.
The system is to be based upon a multiprocessor computer architecture that allows system upgrades through software changes.

The Equipment is to be compliant with the Ontario Healing Arts Radiation Protection (HARP) Act having the following controls, features and/or functionality:

an indicator to alert users when the system is set to operate at an exposure rate over 10 R/min
to have a 30 cm cone separator in accordance with the HARP Act
visual "Fluoro On" indicators;
audio "Fluoro On" indicators;
to have a five (5) minute fluoroscopy alarm;

The Equipment performance is not to be compromised the EM radiation generated by cellular telephones, walkie-talkies or other similar transmitting devices;

General Specifications:
1. The clinical preference is for the Equipment to have as small of a footprint as possible. Such footprint is to take into consideration the following:
 weight of the C-arm station (kg);
 weight of the C-arm;
 dimensions of the C-arm station (h x w x d) (cm);
 the minimum free space required between the x-ray tube and the image receptor (cm);
 depth space of the C-arm (cm);
 orbital rotation of the C-arm (°).
 horizontal travel of the C-arm (cm);
 vertical travel of the C-arm (cm);
 panning motion of the C-arm (°);
 pivot rotation of the C-arm (°);
 weight of the workstation (kg).
 dimensions of the workstation (h x w x d) (cm); and
 the required C-arm clearance around the patient and the OR table.

Ideally the Equipment will be designed in such a way as to allow for operation by clinical staff in either a standing or seated position. Such design is to take into consideration avoidance of exposing clinical staff to potential repetitive strain injuries.
 ease of use to connect cables and not get confused
 motorized vertical travel of the c-arm with capability to travel at least 35 centimeters vertically;
 capability for the c-arm to rotate minimum180 degrees
 capability for orbital rotation of minimum 115 degrees

2. Equipment Design
Bidder to provide details related to Equipment finishes and designs with special consideration to the points below:
 All Equipment edges, handles and knobs are to be smooth and sufficiently concealed or protected to ensure that they do not create bumping and/or injury hazards;
 Ideally the Equipment would have protectors that would prevent cables from being run over, or creating undue safety hazards.
 A secure cable storage area;
 Overhead extensions of the Equipment are to be engineered in such a way to ensure that they not a concern for injuries;
 External components are to be securely affixed to the Equipment;
 Braking mechanism with independent set and release for the front and back wheel sets;
 Ease of movement of the Equipment on a smooth surface floor such as tile, sealed vinyl that would typically be used in a hospital setting

3. Technical Assessment
Bidder to provide specific details relating to:
 Low contrast resolution
 High contrast resolution
 Dose assessment for given image quality

4. Draping
During operation, draping of the Equipment and the components listed below is necessary. In order for draping to be successful, the Equipment mobility and/or operation cannot be impeded.
 Image Intensifier
 C-Arm
 X-Ray Tube
Bidder should provide information related to draping of the above components during operation.

5. Image Quality and Functionality
As a minimum, but not limited to the Equipment should have the following:
 Subject harm and/or the inaccurate recording or display of imaging data is not to occur as a result of Equipment failure or a power outage situation;
 Normal magnification of 6 inches and 8 inches field of view
The Supplier should agree to work jointly with the Hospital to establish image quality protocols. Such protocols will include an image quality baseline at the time of installation, and an agreed upon frequency for the replacement of the necessary component to maintain an optimum system performance.

6. X-ray Tube
Ideally the x-ray tube for the Equipment would be a rotating anode with the following capability and/or functionality:
 Focal spot size(s) for radiographic mode (cm)
 Focal spot size(s) for fluoroscopic mode (cm)
 Ideally the Equipment would allow the X-ray tube and the II to be reversed. The net result is that the position of the tube could be over the table or under the table.
 Heat storage capacity in Heat Units (HU): 1.6 million heat units
 A warning indicator for the user if the heat capacity is getting near its maximum limit;
 An average life expectancy for the x-ray tube of 10 years.
 Removable grids and filters

7. Generator
As a minimum, the generator:
 is to be capable of providing a boost or a high dose fluoroscopic exposure of up to 20 mA;
 is to have a minimum pulse rate for pulse fluoroscopy of 8 pulses per second;
 system is to be capable of providing digital spot or digital radiography mode;
 is capable of providing cine with rate of 30pps;
 is equipped with navigation aids such as subtraction, roadmapping, and landmarking
 is to be capable of printing the exposure parameters, including dose the per exposure;
 is to be capable of storing the exposure & dose data with the image; and
 is to be capable of recording and storing to PACS with the total dose, including units of radiation dose.
The Equipment should be capable of real time reporting, continuous indicators of dose in both kV and mA.

8. Fluoroscopy System
The Equipment functionality should include the following fluoroscopy modes:
 regular;
 last image hold with capability of simultaneous viewing with live fluoroscopy;
 it is preferred to have a keyboard with annotation functionality.

 a reverse position.
 a isocentric rotation.

9. Equipment Workload/Output
 The system workload is estimated at five full days per week, eight hours per day.

10. System Compatibility and Integration
 As a minimum, but not limited to, the Equipment is to be compatible with and interface capable of connecting to the following technology platforms and/or systems:

11. Human Factors and Clinical Assessment
• This equipment is to be used with large animal models
 Proof of Concept - Usability Testing Short listed Bidders will without limitation, be required to provide proof of concept for their proposed solution by participating in on-site (UHN) usability testing and analysis of their proposed system during the period identified as per the schedule in Section 6. Usability testing will involve observation and recording of end users as they attempt to use the system during representative real world scenarios. During each session, audio, video and task records will be collected. Analysis of this data will allow for the discovery of aspects of the system that were difficult to understand, prone to error, inefficient or ignored. This information will be analyzed together with the recorded scenarios of multiple users for each of the systems being evaluated to uncover the optimal solution as well as areas for improvement in each.
 In preparing for usability testing please be aware of the following:
o The application will not be interfaced to UHN’s HIS or with any medical devices
during the usability testing.
o Between 10 and 15 representative end users comprising all three UHN sites may participate in usability testing each of the short listed systems. It is estimated that each user will spend between 60 and 90 minutes usability testing each system.
o Prior to usability testing, templates to be used during the scenarios will be developed in conjunction with the Bidder.
o At the conclusion of testing, each participating Bidder will receive a report with video highlights and recommendations to address any usability issues. Findings will be kept confidential and only shared with the project team.
o Ease of use of the Bidder’s proposed system will need to be demonstrated UHN shall evaluate each proposed system and make its final selection, if any, of the successful Bidder based on site visits; usability testing; and the evaluation of the Proposal.

12. Software
As part of this offering under this RFP:
 the Supplier is to make available to the hospital all the trouble shooting service software tool that the Supplier's team uses for the life of the Equipment at no additional cost.
 the Supplier is to make the service software available to the hospital for the life time of the equipment at no additional cost.
 the Supplier shall notify the Hospitals when upgrades that affect performance become available.

13. Disposables and Accessories
Reusable accessories that require sterilization shall be provided with a sterilization container.

14. Materials Properties and Cleaning
As a minimum, the Equipment, including, but not limited to its surfaces and finish is to be constructed of durable material that is both oxidation resistant and able to resist leaching from repeated exposure to Hospital approved disinfectant. The Hospital currently uses Virox and Cavi wipes.

15. Warranty
 Unless otherwise agreed, the warranty period for the Equipment shall be 2 years standard manufacturer’s warranty that includes all parts and labour for the Equipment, warranty for subsequent years should be priced and payable on an annual basis. This is to include all parts and labour associated with the Equipment. A total warranty period of 5 years is to be provided by the supplier.
 Unless otherwise agreed, all repairs are to be completed on Hospital premises. If the parties agree that the Equipment cannot be repaired on site, the Supplier, subject to the Agreement is to provide replacement equipment for the duration of the complete repair.

16. Warranty Up-Time
 The Hospital’s preference is to have a 98 % "Up-Time" of Equipment. For each percentage point of "Down-Time" due to unplanned maintenance the warranty of the equipment will be extended for 3 months.
 “Up-Time" applies to a total operating time of 365 days per year, 24 hours a day. "Down-Time" refers to the period between the onset of the service problem and/or service call to the Supplier to the time of resolution where the equipment once again meets the quality "benchmarks" as determined at first clinical acceptance and the Supplier's published specifications.
 Maintenance
 The Supplier will provide as part of the purchase the service software licenses for the life of the Equipment.
 The Supplier is to provide access to factory expert technical support for this Equipment to the Hospital.
• The Hospital will require the Supplier to have on site at the Hospital parts known to commonly fail. The Supplier and the Hospital will mutually agree on a list of parts to be stored on site. The Hospital will endeavor to provide within reason a secure location for parts storage.
 All other parts that are not stored on site will be required on site within 24 hours of placing an order with the Supplier.
• the Hospital may elect to purchase parts from a third party distributor in some cases. In such situations, the Supplier will be required to support the installation of such parts. For example, but not limited to, aged equipment where re-furbished parts are purchased vs. net new simply due to the age of the equipment.

17. Reports
• The Supplier is to provide a service report for all Equipment repairs completed during the term of the Agreement. Service reports as a minimum, but not limited to are to contain the following repair information:
• Serial Number, Model No., Description of Repair, Parts required, Cost of Repair, Date of Repair.
• As a minimum, the Supplier is to provide an annual, report detailing the complete repair/service history as outlined above. In addition, the Supplier is to provide a final repair/service history report no later than thirty (30) days prior to the expiration of the Agreement.
• The Supplier will perform a complete re-evaluation of the system at the end of the Warranty Period to ensure that the Equipment performance meets the benchmarks as determined at the initial Equipment installation.

18. Latex Free Products
 The Hospital would prefer to purchase products that are latex free or have a minimal amount of latex to minimize patient exposure to latex in order to reduce adverse reactions and to promote a latex safe environment.

19. Products Containing Mercury
 The Hospital would prefer to purchase products that are mercury free, except when sphygmomanometers containing mercury are required for specified specialized medical practice.

20. Electrical Equipment
 The Supplier should provide the status of the electrical equipment proposed pursuant to this RFP in terms of whether it has been authorized or approved in accordance with the Ontario Electrical Safety Code by a Certification Organization, accredited with the Standards Council of Canada Act (Canada), and whether it bears the Certification Organization's mark which identifies equipment certified for use in Canada. The Certification should be to the standard that is appropriate for the intended use of the electrical equipment at the Hospital’s facilities.

21. WHMIS
 The Supplier will be required to comply with the requirements of the Occupational Health and Safety Act at all times.

22. Cost Reduction
Bidders should provide details on cost reduction incentives.

23. Technical Support (If Applicable):
Bidders should include a two (2) hour response time for telephone, email or remote desktop support during Bidders normal business hours from time of the Hospital’s request. The response time should be less than forty-eight (48) hours for services requiring technician’s on-site repairs or parts replacement. The bidder should make every effort to arrange for replacement parts as necessary within a period of (24) to (72) hours from the initial service contact. The bidder should provide a guarantee that the instrument will not be left unusable for a period exceeding five (5) days so long as it is under warranty coverage.
Bidders should provide a detailed description of the warranty implementation terms and conditions including the geographic location of technical service personnel, the location and availability of replacement components, standard shipping practices for replacement components, and the long-term availability for such components.

24. Product Installation & Training If Applicable – Details:
Please provide details relating to the amount of time required for installation of the equipment and site installation requirements.
Please provide information on product training. This should include the number of sessions, duration, and number of allowed attendees.

25. Other Information:
Please provide any relevant supporting documentation, cost-saving incentives, promotions.
